1) How do I change my DPH Active Directory (AD) account password?

You can change your DPH AD account password from the DPH Intranet or by calling the numbers below.

2) What if I forget my DPH AD account username and password?

You may contact the UCSF IT Service Desk at (628) 206-5126 for further assistance during business hours or the DPH Service Desk 24/7 at (628) 206-SERV or (628) 206-7378.

3) Will my access be locked after multiple unsuccessful log in attempts?

To protect your account, your token will be locked out after 8 unsuccessful log in attempts.

4) How can I unlock my account?

 The account lockout will last for 30 minutes and unlock automatically. If you are unable to log in after 30 minutes, contact the UCSF IT Service Desk at (415) 206-5126.
